Below is some of the information that was emailed to registered families.

Eagle Island Camp Medical and Permissions

The medical paperwork will need to be signed by a doctor but you can also replace it with any similar form or physical that you have sent to the school.  If you do use your own physical form please also fill out the over the counter section of our form and make sure to include any allergies or accommodations your camper may need during our program.  If your camper will be bringing medication with them to camp we will need doctor’s orders and for the medication to be sent to camp in its original packaging. Please feel free to contact me with any medical questions and if needed I can connect you with the RN who will be onsite throughout the summer.

Please return the medical paperwork to Eagle Island Camp, PO Box 304 Saranac Lake NY 12983 by July 15.

The information packet includes some information for you and your camper as well as some forms to be signed by the parent/ guardian/ or caregiver.  These forms can be emailed back to kdearden@eagleisland.org or mailed with the medical paperwork to the above PO box by July 15.

We are offering van transportation for $25/week (fee is waived for camper’s attending on financial assistance).
